I would say its your shifter fork is broken,plus the gear for reverse is worn,causing it to pop out. Check this out.....

This does not deal with the forks,but might be useful. The forks tend to crack and then twist,thus not putting it fully into gear,which then starts popping out and rounds the gear over and it wont stay in at all.If the fork is cracked it will need to be replaced or welded,(i like to add some metel around it when fixing it to make it a little stronger)and the reverse gears replaced.If you have to tear into it a manual will be helpful(i did mine without it just fine),but if you do it without it be SURE you keep everything in order.
