Drive Shaft Vibration

On my 1989 1050, I started to notice my driveshaft vibrates at higher RPM. Thought maybe my driveshaft was out of round, but it looks like the clutch disc has worn through a hole and is not centered on the drive plate. The thing is I just redid the clutch about a year ago and am curious as to what would cause this to wear out so quickly. Any advice?

I'd be looking at the bushing in the drive plate and the end of the drive shaft.
