They can be a PITA to get off. I managed to get the set screw out of mine but the splines on the inside of the lever had worn so I ended up drilling a hole through the top of the lever, put a socket head bolt through the hole and used a puller to get it off. When I put it back together I drilled through the set screw hole, the shaft and out the other side of the lever and tapped it for a socket head bolt. Not pretty but it works.
