In my experience, if your follower had wear on it, that is your real problem. The follower tip wears oval shaped and unless you orient the follower back to the worm gear the same way it was worn, you'll have the problem you have and/or you're going to have to live with some steering play.
It's possible your lock nut has backed off too.
