Any one replace them with slightly longer to put on a lock washer. This is now the second wheel roll of i had this year. I am afraid to crank down any harder than i already do. Currently i have just a washer. Lock tight maybe?20150822_130602.jpg

Front wheel spindle?
Is the bearing good? Lock washer won't do any good with a flat head behind it. My guess is you have a bad bearing..... |

Sorry, forgot the front part. Bearings seemed to be okay, i didnt feel any grinding in them. They are greased pretty heavily. Luckly i was in the grass both times. Also going pretty slow. So when i dropped, i stopped. The first time both bearings stayed on the rim. The last time, the bearings actually fell out of the rim. Both times, making a right turn. I guess i will go left for now on.

All that ever came on them was a large flat stepped washer and a bolt. Are you using a washer that's not big enough and not covering the bearings? If the washer is too small that will happen. Post an ad or order from sponsors to get the correct washer

Check that the center bearing race actually sticks out wider than the bearing, This extra length on the center bearing race is required to prevent the outer race or wheel hub rubbing the Large flat washer and unscrewing it.
J-mech may be right about it being a bad bearing, even if it is smooth and quiet, if it is worn too much it will allow the outer race to rub the washer and unscrew it. Good luck
